Cough and uneasiness.

Chief Complaint A 71-year-old male with a history of gallbladder tumor presents with cough and uneasiness. History He has h/o obstructive sleep apnea He had finished his chemotherapy cycles 1 month back. Today he started complaining of cough, uneasiness and nausea. Investigations CT shows opacities in both lungs. Treatment Advise the management options.

Post chemotherapy pneumonia Adv Check complete blood count - look for neutropenia, leukopenia In this setting infection is usually caused by resistant organism and many times fungal infection Therefore send blood culture and sputum culture Start on higher antibiotic and antifungal agents - example Meropenem and Fluconazole

Evaluate for the active Infective species like, bacterial tubercular fungal infection. Sputum or BAL analysis.

RTPCR for covid 19 Do blood counts, renal function tests, liver function tests

Is this patient on Gemcitabine based regimen? Rule out infection, keep gemcitabine induced interstitial pneumonitis in the back of your mind. If after 3-4 days of antibiotics the patient does not improve consider the above diagnosis
