Cough & SOB

Chief Complaint A 16 y/o male complains of cough and SOB. He was prescribed antibiotics for cough but no improvement. He came back after 1 week with same cough and he also give h/o easy bleeding. History He has h/o diabetes. Vitals BP: 135/65 mmhg, Pulse: 92 bpm, RR: 29/min, Temp=98.8 deg F. Investigations CBC was normal bone marrow biopsy shows B-cell follicular lymphoma. LFT - elevated. SaO2=98% on ambient air Cardiac & Abdomen examination normal. Other investigation shows lactic acidosis. Treatment What is your opinion on the management option?

Cough, fevers , bruising are because of B cell follicular lymphoma Diabetes ,probably ,type1 , has to be treated with insulin Rituximab + chemo cocktails ( R- CHOP) are the main line if treatment for this NHL .

Adv HRCT thorax RFT Coagulation profile. Rule out,Secondary infection.

Needs further investigation and evaluation to conclude and line of treatment.
