Unequal testis in newborn

A day 5 ASYMPTOMATIC newborn has unequal testis. Is there something to worry about??

If there unequal size of testes with the new born babies without any undescended testes there is nothing to get worried. Because if inequalitybin size this can happen in normal condition because lt side testes is smaller in size and at lower label. If size inequality may be due to congenital hydrocele due to presence of congenital processus vaginalis no active treatment required because it may closed in due course of time. Babies may have undescended testes nothing to do at this stage it may also get resolved by one year if not may be planned for treatment. It is too early to call as crypto gonadism.

? Undescended testis. One Testis is not in scrotal sac . Search for it higher up in inguinal canal. If not found, USG abdomen to locate undescended testes . If that side hemi scrotum is small , it is likely to be undescended tests. Feel for end of epididymis. If it is found in the scrotal sac ,but not the testis, it is likely that that side testis is atrophic .
A clinical examination is the most important. If both testis can be felt or palpable in the scrotum- size may be unequal - it is considered normal. But an ultrasound of testis with doppler study should be done if any doubt to rule out neonatal germ cell tumors or congenital hydrocele
If both are pulpable and without uneual sizes of testis a d without either of u descecended..nothing to be concerned abnormal. After few more weeks it would be rivbt time to differenciate Cryptogonadism.

Check if both testes are descended. If both are descended-might be a hydrocele on one side. If that is also not the case-please do an usg

Nothing to worry about unequal size of testes it will not affect anything smaller will be of normal size
