A 38 yrs. known alcoholic patient presented with history of fever on and off with dry cough since10-12 days. CXR s/o consolidation rt. upper and middle zone. Most likely diagnosis.

Rt upper and mid zonal infiltration seen. Possibly CAP/ aspiration pneumonitis. Suspicious left side lower Ribs fractured.
Fluffy infiltration R upper and midzone.?Koch's.? pneumonia.
MOST LIKELY DIAGNOSIS IS TUBERCULOSIS
Rt upper lobe pneumonia...if sputum is not produced then induce with 3%NS and send samples for AFB smear, koh and gram stain and pyoc/s....trt with emperical antibiotics..if pat doesn't improve clinically and radiologicaly then plan for FOB.
