A 37yr F , c/o pain and swelling in the right great toe due to trauma , Hx of Diabetes for the last 14yrs she is on medication - Metformin and Levocitirizine , Surgical Hx - nephrolithotomy was done last yr On Examination- Pallor ✓ Hepatomegaly was appreciated on per abdomen examination Treatment pls??

Cellulitis with infection of great toe with blister formation Usually underneath the blister there is Sara of deep necrosis Adv Strict control of diabetes Debridement of blister with underlying necrosis Fluid and tissue for culture with institution of appropriate antibiotics Find and Treat other conditions such as associated dyslipidemia, hypertension, kidney disease and ischaemic heart disease Check for peripheral vascular disease and treat it accordingly

Pt is diabetic for long period H/o injury to gr toe Discoloration of toe suggest hematoma toe. Xray toe is necessary to r/o underlying bone injury As pt is diabetic careful treatment and investigations are key words as any negligence will cost the toe or sos foot Icepacks Broadspectrum antibiotics Antiinflamatory Rest and raised of foot Strict control of diabetes
