A patient came with black spots all over back side shoulder. Its getting increased. Patient was taking vitamin c500 od Vitamin e Clindamycin ointment but spots are not getting increased. Please dx Please rx also

A 54 year old female presented with widespread skin induration on her face, neck, anterior and posterior chest wall, proximal and distal upper extremities, abdominal wall and thighs. She had a history of poorly controlled type 2 diabetes mellitus. She described a feeling of tightness in the skin and stiffness in the nape of her neck that made it difficult to tuck her chin during swallowing. She also experienced difficulty swallowing both liquids and solids. Unintentional weight loss of 5 kg in a year. She denied heartburn. What do you think is the diagnosis and next step for management?
