What is the Prognosis?

A 63-year-old male was admitted to the ICU with the complaint of uneasiness and dyspnoea. He was discharged from the hospital after around 10 days and was under observation in another hospital where again he started complaining of uneasiness and his situation start getting deteriorated with low BP. Discharge summary from the hospital is attached. Please give your valuable opinion on the prognosis.

Patient is a case of Severe LV dysfunction And needs medical optimization of therapy Since patient has non significant CAD no need for revascularization at present And also since EF is 35 needs ICD or CRT D Depending on ECG findings these patient do fairly ok on medical therapy if treated well provided renal function is reasonably ok should recieve ICD for prevention of SCD

Lvef 35 Known pt of cad with heart failure Probnp Admit under cardiologist

Restrict liquid intake. Continue Diuretics and Sacubitril valsartan combination. May add l carnitine, sos dobutamine therapy. Otherwise same treatment to continue.

CAD with severe LVD Cardiologist will justify the pt .

Optimise the treatment and add heart failure management he will improve

Pl post the lab reports & all.the cardiology work up reports with x ray chest

Acute MI with CHF ICMP with low EF Must meet any cardiologist

Just add Mankind Placida 1tab BID & watch

As pt is Diabetic with CHF and MI Refer to Cardiovascular team for required surgery / management otherwise prognosis is not good.
