A 35 year old obese Woman having history of PCOS presented with complain of Upper Back pain since 7 year. On examination nape of the neck was tender with forward leaning body posture. Pain also present on left arm and left chest. X- ray AP and Lat view of neck attached. CBC and ECG normal. Diagnosis and management ? Chief Complaints Upper back pain - 07 year Occassional Chest pain -07 year Left arm pain (Non radiating)- 06 year History History of twisting injury to neck in child hood. PCOS Vitals NAD Physical Examination Tenderness at nape of neck. C-6 C-7 spine tenderness. Chest- Not suggestive CVS- Not suggestive. Investigations X - ray - Attached ECG- WNL

Clay Shovelers fracture (Chronic case). See typical history. C-6 vertebral spine fracture. Patient responded to analgesics and physiotherapy. Also she was advised for posture correction exercise.

Cervical spondylitis may be combined with frozen shoulder. Chest pain due to posture condition. Physiotherapy management may be helpful. Traction cervical and thoracic will helpful to minimise pain.

It's case of functional scoliosis...leading to change in other physiological aspect the body..vitd3 is very less.it is might be congenital case of metabolic disorder in childhood like rickets .

The IV disc space between C 4-5 is slightly reduced.?? Early spondylosis.

May be due to Upper cross syndrome (posture related) chest pain may occur due to improper rib mobility .. Need physiotherapy assessment and treatment And weight reduction with aerobic conditioning
