47 year man pain abdomen nd chest pain known htn dm

A 72 year old farmer is seen for a check up prior to inguinal hernia repair. A chest xray is requested as he has a smoking history. An interesting incidental discovery is made. Note that the image contrast has been manipulated to best show the abnormality.

45 year old gentleman with history of on and off fever with chills from past 2 months. He also gives history of productive cough since past 1 month. x ray chest and routine investigations done and have been added for reference.
