1. Admit the child 2. Clean the wound with tap water throughly, may use betadine mouthwash as well. 3. Give human rabies immunoglobulin 20iu per kg body weight, half through infiltration and half through im at upper arm with in preferably in 48hrs. 4. Give rabies vaccine ASAP 5. Start iv antibiotic covering staph and enterococci. 6. Minimal surgical intervention especially bleed control 7. Secondary suture may be done after 3 - 4 days. And cosmetic repair may be done at later stage I. E. Atleast 10 days.

As per WHO recommendation..."as much as anatomically feasible of the RIG should be infiltrated around the wound. Any remainder should be injected at an intramuscular site distant from that of vaccine inoculation e.g. into the anterior thigh"

Anti rabies vaccination along with RIG half the dose locally and the rest im. Hospitalise Start tt inj Ivantibiotics Meticulous cleaning and dressing. Special attention for nutrition and rehydration. Watch for vitals Anti inflammatory and analgesic for pain and inflammation Refer to COSMETIC SURGEON for repair

Interesting case doc, since you are the plastic surgeon yourself, how did you go about with the case.. Would like to have more in sites on the case.

Wash with soap & water. Inj TT if dpt not taken at 4.5 years, rabies immunoglobulins ( local), ARV as per schedule . No suturing. Amoxclav and ibugesic . Local t bact ointment

Thank you all.. we did admit the child, gave ARV and immunoglobulin (locally as well as IM).. after 3 doses of ARV, Pt was taken to OR for management of defect.

Anti rabies vaccine, Immunoglobulin locally , IV broad spectrum antibiotics, Primary closure of wound .
