Dyspnea on exertion

Chief Complaint A 54 y/o male came with dyspnea on exertion for almost 1 month. History He is non smoker and drinks no alcohol. No history of heart disease. Vitals Vitals are wnl except Pulse of 40/minute. Investigations Lungs - clear. Chest x-ray shows no pathology. complete heart block. Labs: TSH: 2.2 WBC: 5000/mm3 Platelets: 149,000/mm3 Hematocrit: 45.5% ESR: 8 Potassium: 4.2 mmol/L Treatment What is your opinion on management?

DOE is caused by bradycardia and complete heart block Complete heart block causes asynchronous contraction of atria and ventricles, this result in decreased diastolic filling of heart, this results in decreased cardiac output Bradycardia, further decreases the cardiac out put This results in breathlessness Insertion of pacemaker will improve heart rate and it will cause synchronisation of atrial and ventricular contraction this will improve cardiac output
