Right posterior occipital headache & low-grade fever

A 32-year-old woman presents with onset of a severe right posterior occipital headache and low-grade fever. Aggravation- bending forward, cold air Amelioration- pressure Her physical examination reveals an area of erythema and swelling posterior to the right ear and a nonmobile tympanic membrane in that ear. Appetite, thirst- normal desire- salty things aversion- no stool- constipated urine- normal thermals- hot Menses- normal What is the most appropriate next diagnostic step?
