PARADOXICAL LOWER HBA1C IN OLDER PATIENTS WITH T2DM WITH COMORBIDITIES

Dear my friends and fellow Colleagues, Here is an interesting update on Lower HbA1c in older patients with multiple in patients suffering from T2DM. HbA1c Levels are paradoxically Lower in Older patients with Multimorbid Patients With T2DM. How ever treating elderly or older T2DM patients is always a challenging task because of multiple associated comorbidities like HTN Dyslipidemia DKD/CRF Malignancy Recurrent Hypoglycemia Cognity Impairment. Reference McCoy RG et al, Paradox of glycemic management: multimorbidity, glycemic control, and high-risk medication use among adults with diabetes. BMJ Open Diabetes Res Care. 2020;8(1):e001007. Regards and thanks, Dr Sepuri Krishna Mohan.

This may mean , In older patients ,with multiple comorbidities, Fasting and PP blood sugar assume more importance in comparison to HbA1c in assessing diabetic control However, international opinion is in favour of keeping HbA1c higher in elderly with comorbidities , especially people with life expectancy below 5 years to 7.5 to 8.5 % . This will need some thinking in planning diabetic controlling view of this new revelation.
