History Study individual was symptomless before 3 years gradually emerging symptoms like tremors on right hand is observed. Then she approached to civil hospital and started with TAB LCD (Levodopa +Carbidopa) ½-0-½ TAB BEXOL (Trihexyphenidyl/Benzhexol 2mg) ½-0-½ TAB Pramipex 1-0-1 TAB Telmikind 80mg 1-0-0 HISTORY OF PAST ILLENSS – K/C/O- HTN under treatment since 4 years Tab Telmikind 80 mg 1-0-0 ( After food) P/S/H- LSCS (2002) FAMILY HISTORY – Nil. MENSTRUAL HISTORY – G2 P2 A0 L2 D0 Menopause 10yrs back Allergy – nil Addiction – nil Diet - veg Physical Examination Subjective- Tremors at rest Right hand weakness , rigidity, stiffness Involuntary movements Objective- No specific test exists Dopamine transporter computed scan (DAT) scan Investigations ASHTAVIDHA PARIKSHA - Nadi – sarpa-gati Mal – samyak Mutra – samyak Jivha – saam Shabda – spashta Sparsha – anushnasheet Druk – aaraktavarni Akruti – madhyam O/E – GC- Fair T- Afeb P- 68/min BP-160/90 mm of hg S/E- RS - AEBE - Clear CVS - S1S2 N CNS - Conscious and well oriented P/A – Soft , non-tender P/H- Bowel – clear Bladder – clear Appetite – normal Sleep- normal Thirst – normal Diagnosis She was having c/o- Dakshin hasta kampa Dakshin hasta kriyalapata Dakshin hasta daurbalya Dakshin hasta Aniyantrit kriya since 3 years Patient came to opd with prediagnosed by an allopathic physician as PARKINSONISM. As Ayurvedic point of view this symptoms can be shown in KAMPAVATA Management 27/02/19 c/o-Tremors on Right Hand Kampavatari rasa 1-0-1 (After food ) Vishatinduk vati 2-0-2 (Before food) Tab Neurivive 2-0-2 (Before food) Zandopa powder 1 tsp-0-1 tsp Dashamularishta 2½tsp-0-2 ½tsp +Ashwagandharishtam 2½tsp-0-2 ½tsp (1 cup of warm water ) Varuni Taila 5ml -0-5ml (with food) F/U on 8/3/19 Treatment - same as above C/O - Tremors on right hand decreases
MODE OF ACTION The two features i.e. Stambha and Cheshtahani exhibit avarana of kapha in the normal functioning of vayu. Hence on these grounds it could be understood that the action of Varuni Taila is mainly on removing the kapha avarana. Tikta rasa and katu rasa have been attributed with pachana and srotoshodhana activities, which in long run are not suitable in vata vyadhi chikitsa. Indravaruni, which is basically virechaka as well as deha samshodhana dravya, which is uncommonly used due to teekshna guna pradhanyata. Neuro inflammation is one of the driving forces in the pathogenesis of Parkinson's disease . Neuroinflammatory mediators have a profound action on CNS cells that differently affects the progress of inflammation and neuronal death.

Roots of Citrullus colocynthis showed to have anti-inflammatory activity. Recent evidence indicates that accumulation of protein filaments in Lewy bodies actively takes part in the degeneration of neurons. This accumulation of protein can be considered as Avarana of Kapha causing aggregation of vata further leading to Dhatukshaya i.e. loss of dopaminergic neurons.

Seeing to the promising result of Varuni Taila this legacy of Ayurveda needs to be practiced on a large scale so as to have strong affirmative concrete evidence. Although the anatomical conidia of degeneration of nerve cells in the brain is not possible , but physiological parameters can be rectified kept under control. Multicentre studies required .

Intramuscular epinephrine 0.3 mL of a 1:1,000 concentration up to every 5 minutes in adults or 0.01 mg/kg in children up to a maximum dose of 0.3 mg Intramuscular promethazine Intravenous fluids Systemic corticosteroids may be used to prevent the delayed-phase reaction in acute anaphylaxis and to prevent/treat associated angioedema and lower airway inflammation. IMMEDIATE MANAGEMENT OF Non–IgE MEDIATED REACTION NON SERIOUS REACTIONS : antihistamines SERIOUS REACTIONS… STEVENS JOHNSON SYNDROME : The use of tapered doses of systemic corticosteroids is not uniformly practiced by all specialists in drug allergy.Oropharyngeal hygiene and gargle solutions, as well as eye care should be ensured. TOXIC EPIDERMAL NECROLYSIS… Skin care, eye care (sterile eye management, topical corticosteroids), adequate hydration and nutrition and respiratory care are paramount. High-dose intravenous immunoglobulin (IVIG 1 g/kg/d for 2 days) has been used at various centers with generally good outcomes, especially in improving skin re-epithelialization. However, the evidence remains controversial, and the original hypothesis on the anti-apoptotic effect of IVIG now does not appear to be so. Other immunosuppressive therapies, including cyclophosphamide, plasmapharesis and systemic corticosteroids, have not been found to be uniformly useful. Recent interest has re-emerged on the possible benefits of cyclosporin in these patients. DRUG INDUCED HYPERSENSITIVE SYNDROME The use of tapered doses of systemic corticosteroids is not uniformly practiced by all specialists in drug allergy. DRUG DESENSITIZATION.. Desensitization is a process in which the drug to which the patient is allergic is administered to the patient in small incremental doses to induce a state of temporary tolerance to the drug. A 45-year old male developed this rash after taking clindamycin and metronidazole for a tooth infection. The rash not pruritic and it started on his abdomen after it spread and become confluent (this is 3rd-day of rash appearance). He also has a fever up to 38 degrees Celsius. In a clinical exam, I found nothing but this blanchable rash on thorso and face, and a small purpuric rash on his legs. I gave a diagnosis of cutaneous drug reaction The interesting this is that the rash is most prominent in the area where T-shirt was in contact with the skin.
