F/58 Complaining of hyperpigmented lesions over both side of neck r quite some time.non itchy.pt is normoglycaemic.diag n TT plz



Dr. Samaredra, It's no doubt POSTPIGMENTED INFLAMMATION . PPI This pigmentation is caused by exoallergens may be metal or perfumes . So the end result is pigmentation . Treatment : Avoid contact allergens . Moisturisers contains aloevera morning . Night skin lightening creams L- glutathione daily after BF will give good result .

Other area of hyperpigmentations should be checked like knuckle pigmentation and axillary pigmentation. If RBS is normal then malignancies should be ruled out. History topical or systemic medication might help in reaching diagnosis. Ddx: # acanthosis nigricans(rough surface) #Exogenous ochronosis (look for h/o application of topical drugs) #Poikiloderma of civatte(follicular accentuation and triad of atrophy, HP, and teleangiectasias but commin after menopause) # lichen pigmentosus( face also would hv been involved) #erythema dyschromicum or maturational dtschromia # drug induced hyperpigmentation.

possibly post-inflammatory hyperpigmentation May be allergic contact dermatitis or Irritant dermatitis to metal ornaments or sprays or deodorants DD acanthosis nigricans only thing it is patchy DD melasma DD photodermatitis DD lichen planus pigmentosus


Since it is non itchy ACD is unlikely . Acanthosis nigricans is likely . DD Post inflammatory hyperpigmentation
