Fever and malaise

Chief Complaint A 69 y/o male presents with fever and malaise for 10 days. No other complaints like cough, breathlessness, or chest pain. History No history of any underlying combordity. Family history unremarkable. Vitals BP- 130/80 mmhg, Resp rate- 18 b/m, HR- 86 b/m, temp- 102.4°F. Examination Physical examination is normal. Investigations WBC: 10.4 x 109/l, Hb: 11.4 g/dl, platelets: 226 x 109/l. Chest x-ray shows complete opacification in the left lung. CT shows dilated esophagus involving left hemithorax and pleural effusion. Treatment What is your advice?
