Rt sided hyperinflation Left sided volume loss with mediastinal herniation towards ipsilateral side. Intestinal loops noted in left lower zone. Possibly secondary to pulled up diaphragm. Can do USG thorax. Evaluate for Koch's relapse or superadded infections. Treat as COPD emphysema.

Left lung significant loss of volume, because of old tuberculosis There is gross shift of mediastinum to left side Therefore are few area is calcified lymph node in right parahilar region Left dome of diaphragm is pulled up because of scared left lung Please send Sputum for AFB, gene Xpert and TB culture to rule out active disease
