A 3 yr old girl with this lesion on right side of the nose for 5 days Initially like very small black spot ,then gradually increase and it produce white serous discharge No h/o any piercing No h/o fever no h/o itching No h/o similar complaints in other areas of body

Shape of lesion with hyperpigmentation indicates metallic allergy in contact area. So evaluate the correct history and treat accordingly

Bacterial skin infection is extremely common in atopic dermatitis. This is usually with staphylococcal or streptococcal bacteria (see staphylococcal skin infections and streptococcal skin infections). Infection is in part due to breaks in the skin from very dry, split skin and from scratching the itchy areas. Do not squeeze the pus out of the abscess yourself, because this can easily spread the bacteria to other areas of your skin. If you use tissues to wipe any pus away from your abscess, dispose of them straight away to avoid germs spreading. Rx Tab acetomazine 250mg BD Tab levocetirizine 5 mg TDS Antibacterial ointment cream Clean skin warm water 5 time days Take vitamin C

Differentials should include 1. Porokeratosis 2. BCC Give a trial of antibiotic (though i guess won't respond) If it doesn't respond.....Go for 2mm skin punch biopsy and send it to a dermatopathologist Then proceed accordingly

Start with cefixime syrup or agumentin duo syrup tid 2.5 ml . Fucold syrup 3 ml BD. And t bact ointnent bd aplicatiom ..clean the area with warm cloth and then aply t bact. . Could b bacterial infection
