Male patient comes in opd with complai. Of mild abdominal pain,weakness and headache since two days Chief Complaints Headache Weakness Sleeplessness History Hitory of taking alcohol since 20 years Vitals Bp: 130/80 Pulse 120 Physical Examination Weight 85 App: low since 5 days Sleep: reduce since 4/5 days Bowel normal Bladder normal No history of vomiting No history of trauma No history of dm No history of htn Investigations Cbc:esr 30 Kft: NAD Lipid profile: NAD Lft: Ast:170 ALt: 180 GGT: 745 Diagnosis Migraine?? Anxiety?? Hepatic insufficiency?? Management Need to be discuss Need valuable opinion
All hepatic enzymes are increased. But maximum increase is with GGT Hepatitis , most likely alcoholic hepatitis Viral hepatitis profile need to be done . RTPCR for Covid as routine Abdominal ultrasound Suggest URSEDEOXY CHOLIIC ACID Hepatoprotectives Alcohol abstinence Psychiatric consultation
Headache, Fever and tachycardia point to infection as cause Kindly rule out Covid 19 by doing RT PCR test
