Hello everyone i want suggestions regarding the case A elderly male presented on 3rd day after dog bite with fever he has not taken any ARV till now Local examination Patient has a mark of one teeth of dog with cellulitis above near with serous fluid oozing from wound Comorbidities He is a known case of DM type 2 and not taken medicines regularly So what can be the management of this pt

Certainly treat as dog bite with secondary infection Wash the wound under clean running water Dry infiltrate anti rabic vaccine in wound rest to be given intramuscular in deltoid Pt is diabetic hence needs strict control so edjucate the pt and keep on appropriate OHA Broadspectrum antibiotics like amoxyclav625mg 1bd Or tan Lenazolinid 600mg 1bd Antiinflamatory Follow 5inj shedule of ARV

we need in vitro studies to know the efficacy of the herbs in killing the virus. The other fact is that almost 85% of dog bites are from uninfected dogs, so the efficacy of traditional therapies being used in rural areas is questinable and is not scientifically established. I generally refer all cases to govt hospital as in addition to the vaccine they require local infiltration by anti rabies serum and also injections of immunoglobins in severe cases( 2nd degree and above).Hope some does a research on the topic and establish its efficacy scientifically.

Clean wound and cover it with antibiotics ointment, no dressings Inj TT Inj AR immunoglobulin ,IM and locally in and around lesion . Inj ARV ,5 injections schedule Antibiotics orally . Tight control of DM

Strictly he should have to take ARV as per advice with controlled DM & for cellulitis Antibiotic with anti inflammatory with daily wash with NS
