Lip eruption lower 4day. No fever,no pain ,49 yr male, ?herpes labialis,cold sore,is oral antiviral indicated , please eloberat.thanks

Mild herpes labialis. No special treatment required in your case as no fever and other symptoms are absent. If needed,Can use topical antibiotics to prevent secondary bacterial infection.but this decision is made only after physical examination of the blister.

Doctors prescribing oral antiviral on 4th day consider two things in account. (1) When infection is severe or vital structures are involved... to decrease the viral load (2) With some kind a hope of herpes not recurring or less frequently recurring. Topical antiviral is controversial. I would have emphasized more on secondary infection.. doxycycline and topical mupirocin / fusidic acid.

Its herpes labialis. Acyclovir effective only if started within 24 to 48 hrs of eruption of lesion .otherwise its of no use . Better to focus on pain if present n secondary bacterial infection .
