63 y/o male presented with the chief complaint of a palatal lesion that was of about one month’s duration. The patient stated that about a month prior, he had started to notice roughness in his palate. No pain or numbness, no symptoms related to swallowing or speech difficulty. Examination shows no neck nodes or any other symptoms. Lesion was irregular, was flat in some areas and slightly exophytic in others. The color was not uniform. PMH is significant for rhinoplasty in 1985, malignant melanoma of the skin in 2008 successfully treated with surgery. No history of smoking. Could it be Drug-associated hyperpigmentation or Systemic disease or anything else?

30 year old male. no medical history. uncle and 3 first cousins were diagnosed with celiac. patient reports taking a blood test for celiac a few years earlier which turned negative. the patient started feeling a tingling in the mouth after eating a red grapefruit (no known issues with grapefruits) and in the next 2 days the sensation worsened and aphtae showed up on tongue and upper and lower inner lips. patient complains of painful aphtae, difficulty in eating and speaking.
