How to treat broncho spam in case of PE with hypoxia causing wheezing..
Bronchospasm in PE - will be relieved once PE itself is treated with 1.Diuretics 2.Warfarin or LMWH or fondaparinux 3.Thrombolyric therapy 4.Oxygen support Bronchdilators may have some supportive role in addition to above main treatment
PE if pulmonary edema, diuretics with nebulized ICS LABA combination is helpful. If pulmonary embolism, no role of bronchodilators as pathology is intravascular.
Diuretics... Bronchodilator...Steroid nabulizer/ Inhaler..

76 years old male - k/c/o CAD -HTN- DM Operated for right lower limb both bone # . Nine days back - all measures were taken post surgery and diabetes was controlled - clexane stopped after five days due to rectal bleed . yesterday night patient c/o breathlessness - Spo2 - 80% RA - B/l crepts present - patient started on CPAP - patient stabilised - today morning around 7 am patient arrested abg showed mixed acidosis - patient reverted and intubated - started on inotropes still BP is 95/66 mm hg - Cbg- 346 mg Dl - spo2 - 97 on 60 Fio2 - ? Differential diagnosis ? Further management ?
