Hx For 3 weeks now, patient has been noticing these lesions on left nostril, these lesions occur and recur. Dx? Rx?

Can be a herpes infection aswell. Pain, fever lymphadenopathy, self limiting point towards a herpes infection. A long duration itching, non increasing size, and history of constant contact may point towards contact dermatitis

No itching, no lympadenopathy, no fever. Patient was earlier diagnosed with contact and nickel dermatitis.

Allergic contact dermatitis low potency topical steroid cream will be helpful along with systemic antihistamine drugs.

Seborrheic dermatitis or contact dermatitis

Herpes simplex. Acyclovir oint locally and tab acyclovir 800mg qid for 5days.
