50year male, smoker presents with cough, fever, dyspnoea at rest, right sided pleuritic pain since almost 20 days. ESR WAS 98 WBC 19000 RBS 100 CREATININE 1.2 WHAT'S THE DIAGNOSIS? GIVE DETAILED MANAGEMENT APPROACH.

Hydropneumothorax...icd placement and sample should be sent for AFB CBNAAT and C/S. till then empirical antibiotic should be given.azithromycin or a combination of pipercillin and tazobactam is a good option. Wait for the reports.

Empyema... or Hydropneumo thorax. Need to do ICD insertion and send fluid for culture..
