Hyponatremia

A 80 yrs old male cad post ptca ,lvef 35,admitted here in state of dullness,altered sensorium,his bp was very low,pedal oedema,malnourish,his sodium was ver low 98,serum osmolity low 215,urine osmolity near normal 121,urinnary sodium low 5,potassium 5,tlc nlormal,urea,creatinine normL,uric acid low 2,albumin low 2.2,mild pleural and ascitis,TSH normal Plz tell me weither hyponatremia is due to hypvolumia,hypervolumea,euvolumia or due to cortisol deficiency

He is kco cad post ptca and in lvef As per reports pt is in hyperkalemia and hyponatremia likely due to hypervolumia Probably diabetic bsl and KFTS not given Picture seems to be DKD

K/c-CAD post ptca lvf. Anaemia with hypoprotenimia. Hyperkalemia. Hyponatraemia Rx-1-Antianginal drugs. 2-oxygen inhalation stat. 3-Tab ecosprin 75mg 1od. 4-inj deriphyllin+inj lasix iv stat. 5-Tab aldosterone 100mg 1×3 6'-macprost poweder with milk tds. 7-cap vitcofol hb 1od. Follow up and monitor the case as per recovery

Most likely patient is taking diuretics and hyponatremia is drug induced.
