Time old bronchodilators used from a long time as LABA and LAMA Very useful in cases of copd and chr asthmatics with short acting salbutamol or salmetrol With albutrol i have my reservations Yes while using the practioner supposed to know its adverse effect as it belongs to atropine derivative hence its consequent complication like acute bronchospam or incresed intraocular pressure in closed angle glaucoma pts so also when inhaled as rotacap leakage in eyes can cause anisocorea Infact idiosyncrasies and sensitivity as with other molecules But in heridetory and chronic cases very useful even in chr allergic rhinitis also So my advice is clinician should go through its literature beforehand Incombination with budenoside its results are encouraging

elderly lady,chulha worker,presented with chronic cough and dyspnea,which is increased since a month.non dm,no major illness in the past.sr creat 1.2,sputum AFB negative.RV neg. o/e bil wheezing with reduced breath sounds. HR 110/min,spo2 44%,no cyanosis clubbing. BP 110/60 mmHg.with 1/2 lit oxygen,spo2 remains 96%. plz suggest further management.
