Primary suturing is first choice However outcome depends on The quality of skin flap elevated, a) was it crushed during injury? If it was crushed it may not survive. b) Is the any fat attached to the elevated skin flap? If fat is denuded and it is thin , it is likely that it may necrose If the quality of native son flap is poor them it is better to excise it and raise another healthy flap and do primary suturing

Days of injury not mentioned here. If primary suturing possible without tension then can go for it after thorough toileting of wound. Otherwise SSG is next option.

After wash ..Primary suturing after undermining to avoid tension..

Primary Suturing

Primary suturing

The wound looks clean but in any case do debriment take culture and sensitivity, and if the hanging skin covers the wound do primary suture ,it does not look as if he has lost skin so go ahead and try suturing. Dress with bactigraa ,it will facilitate ease of further dressing. Must give tetenus inj.

Slice injury of upper 1/ 3 posterior part of left leg. Primary suturing is the first priority. It is difficult to get the upper margin of the skin which has been retracted down. As the margin is a thin there is every possibility that stich may give way.
