Management I prescribe Tab:Deflazakort 6 mg bd Tab:Rutoheal one Bd Tab: Aceclofenac 100 mg bd Tab:Shelcal d3 one od with milk All above med for one week patient feel relief but she is anxious about health What I can do for follow up
Sounds like inflammatory arthritis You brought Remission with anti inflammatory medicines Now she need long term treatment to prevent Relapse Better do it in consultation with a Rheumatologist Rule out other medical conditions that can mimic inflammatory arthritis
Suggest CRP , Anti cyclic Citrulinated protein ( ACCP ) , ANA ( to rule out SLE ) Ultrasound exam of ankle joint to confirm synovial thickening and joint effusion. Follow with DMARD like HCQ, sulphosalicylic acid , methotrexate etc. Steroid can be tapered off. Vit D and calcium supplements. Rest , physiotherapy,

63 y o female presented with painless lesion posterior to tongue right side. Recent hx pneumonia following intubation for elective procedure. 3 years previously resection parapharyngeal pleomorphic adenoma right side; 5 years ago removal pleomorphic adenoma lower lip. MRIs up to one year ago negative. Remote tonsillectomy. Seronegative rheumatoid arthritis x 12 years. Current meds: etanercept 50 mg sc q 14 d symbicort 200 mcg inhaler bid atrovent prn fish oil 3g/d vit D 50,000 U/week Do you have any initial impressions? I hope to assuage her anxiety if this appears most likely insignificant/benign. Thank you very much.
