essential hypertension
Lady complaining of nausea since 15 days,esp since evening.. heaviness of head and body, is on already on light and frequent meals, temporary relief with antacid and antiemetic,still uneasy on and off,vitals stable only blood pressure was 155/92 on checking. USG attached please help with the treatment.

A 52 Y/M with complaint of Intense itching with burning sensation since 1.5 yrs Releived by cold bathing Patient is diabetic hypertensive since 4-5 years Tab Metformin sr 1000 bd Tab telmisartan 40 od Tab chlorthalidone od Tab atenolol 25 od FBS and PLBS are undercontrol Bp remain consistent at around 110/70 Patient is not taking any multivitamins History:- itching started abruptly without any systemic complaints, and initially there were macupapular rash and FOR THAT I GAVE HOMOEOPATHIC MEDICINE HEPAR SULPH 200 THE RASH WERE GONE BUT ITCHING REMAINED Took treatment from dermatologist but to no avail ARBs have a side effect of Urticaria shall i discontinue telmesartan Guide me with diagnosis and treatment Update 11 sep 2019 I stopped telmesartan 40 and patient himself took amlodipine 5mg which he was prescribed prior to telmesartan Now update the bp charts and the skin pictures Update 8 nov 2017 These lesions developed with shedding of skin Photos have been uploaded

Dear Drs... This is a 40yrs female case of diabetic foot ulcer. diabetic since 5 yrs.blood sugar levels are 130fasting and pp 180. obesity nature. house wife. having two kids with normal delivery. hypertension observed as 140/100 using Aten 50 mg . Thyroid also present and using eltroxin 50mg 1 tab per day. heriditary thyroid problem observed. since 1month her little finger had ulcer and didn't heal. she craves sweets and curd.sleep for 6to7 hrs . slightly disturb sleep. night time two times urination. appitite low. please suggest the way of treatment.
