Left-sided chest pain
Chief Complaint A 45 y/o male presents with severe left-sided chest pain which is radiating to the shoulder. He had 1 episode of vomiting when the pain started. The pain started when he was doing cardio in the gym. History No past medical or surgical history. No family history of heart disease. He is nonsmoker but drinks alcohol on alternative day. Vitals Vitals are wnl Invesstigations Chest xray shows Left-sided pneumothorax and pleural effusion Hb: 15.8 g/dL WBC: 11,500/mm3 Neutrophils: 89.2% Platelets: 82,300/mm3 Sodium: 143 mmol/L Potassium: 3.6 mmol/L Chloride: 105 mmol/L Bicarbonate: 25 mmol/L Blood Urea Nitrogen:19 mg/dL Creatinine: 1.8 mg/dl Total bilirubin: 1.4 mg/dL Albumin: 4.8 g/dL Treatment What are your views on management?

Left HYDROPNEUMOTHORAX Needs further investigation and evaluation to conclude particularly pleural fluid analysis and ruled out ICD and A RF. Medical management. Regular monitoring and constant evaluation required.
Left hydropnemothorax ARF Slightly raised bil. Needs left ICD. Evaluate for low platelets e.g. Dengue. Pleural fluid analysis. Medical management accordingly.
Left sided hydropneumothorax , following stressful exercise. HRCT to confirm . Thrombocytopenia has to be explained and corrected. Tension pneumothorax has to be ruled out. ICD on the left chest Left pleural effusion aspirate for AFB AND CBNATT Investigations for thrombocytopenia. and renal failure
Sudden onset pneumothorax indicate underlying lung pathology. Put ICT and collect pleural fluid for cytology and malignant cells. Review with reports
POSSIBLY SECONDARY. TO DENGUE.. NEEDS FURTHER. EVALUATION

4 year old female patient.. history of low grade fever on and off since 1 month.. fever increased since 3-4 days.. also history of cough and cold since one month with respiratory difficulty of 3-4 days duration.. patient presented with severe respiratory difficulty of 4-5 hours duration... coarse crepts bilaterally all over lung fields.. spo2 70% on room air.. dull and drowsy.. x-ray and blood reports are attached... what can be the DDs....

60 yrs old female presented with dry cough, rt sided chest pain and mild fever with chills since a week. not asso with anorexia wt loss. rt basal breath sounds reduced. hemodynamically stable. Pleural fluid tapped diagnostically and further planned ICD. turbid yellow colored 600 ml fluid was drained out on day one, no malignant cells detected, RV neg. CXR, CT thorax and Pleural fluid reports attached. plz comment whether AKT or only antibiotics.

18 year old male complaining high grade fever with pain chest since 2-3 months associated with cough.. Appetite is reduced and pt become generalized weak.. Also complaining diarrhoea infrequently.. He was working as a water supplier house to house.. No family history of as such relevant disease.. . He hv quit smoking.. Tlc count is 8800..pls give ur valuable opinion regarding diagnosis and treatment... Thank u
