Lower back pain for 2 months.

Chief complaint A 54 y/o male presents with lower back pain for 2 months. Description The pain was non-radiating that worsen with movement and restricts his daily routine. No complaint of fever, urinary incontinence or numbness. Vitals BP raised: 150/90 mmhg, all other vitals are normal. Diagnosis Can you tell the diagnosis and tmt.

Could be an infiltrating lesion L2 level Myeloma and secondaries a possibility CBC esr Urine bence Jones protein Serum protein electrophoresis Skull xray lateral view PSA Usg abdomen If results are inconclusive,PET scan May need CT guided biopsy for final confirmation

* Port's spine L2 stress # or body pathology. Needs further investigation and evaluation to conclude and line of treatment.

Modic III changes. Pl investigate further

suspect secondaries send for biopsy and posterior stabilization with pedicle screw fixation
