Diabetic Foot??? What next?
Male Diabetic with foot pain and ulcer in leg... since 2 months... came for further treatment What is next plan? What type of evaluation needed?
Diabetic foot Nonhealing ulcer Necrotising ulcer Needs extensive debridement and off loading of weight Irrigation with h2o2 and NS Dressings with placentrax gel Tab lenazolinid 600mg 1bd Needs to assess circulation in foot But may need Amputation BK
Chronic trophic ulcer ,over 1st metatarsal head , diabetic ulcer , Hansens to be ruled out. Assessment of control of DM. Tight control with insulin . Check for anesthetic foot, due to diabetic neuropathy. Arterial color doppler to rule out ischemic limb . Xray foot to rule out bone involvement. Debridement, dressings , rest , antibiotics, vitamin B1,B6,B12 , special foot wear to redistribute weight bearing Patient education.

Gangrene of foot due to uncontrolled diabties. Sugar control + surgeon opining Antibiotics and chemotrypsin for wound managment
Initially it needs debridement followed by daily dressing e ointment like Ampucare rest antibiotics , sugar control such be done

70 yo man k/c/o dm & htn since 8 yrs. recently diagnosed with diabetic retinopathy. has on & off pain & swelling in rt foot phalenges only which comes back after stopping nsaids. x-ray shows no fracture.uric acid normal. 2d echo,Doppler right lower leg & ecg was done. serum creat,hgt, hba1c,lipid profile within normal limits.please interpret ecg n correlate with Doppler study & suggest further treatment
