A male aged 59 years who is a chronic smoker Chief Complaints Gradually increasing dyspnea Productive cough Respiratory distress Orthopnea (x 1 month) Physical Examination Resp - decrease air entry in whole of right hemithorax Investigations Pleural fluid analysis - Glu - 34, Pro - 4.5 , Alb - 2.7 LDH - 564 ADA - 23

Massive pleural effusion seen on rt side with mediastinal shift towards contralateral side. Adv therapeutic pleurocentesis with ICD. Medical management and further pleurodesis if recurrent and non Infective or neoplastic in nature.

Lung carcinoma Adv Therapeutic aspiration of pleural effusion Look for malignant cells in pleural effusion CT scan of chest to look for underlying pathology High albumin indicates it's exudate

Right sided massive plural effusion with mediastinum shift to left side.. Hyperlucent left sided lung .. Go for HRCT for better results.. Get the report for plural fluid analysis for cell count and differential count. Therapeutic plural tap and treat accordingly..
