Concluded Case

Middle eged male had loose stools with intermittent bleeding from 3 months associted multiple joint pain . No history of fever , weight loss or abdominal pain . Colonoscopy done , poctutes of left left side colon shows these changes . Diagnosis and management?

It was a case of Ulcerative colitis and started on Mesacol tablet with enema along with oral steroids ..

Multiple ulcerative lesions seen on colonoscopy. Associated multiple joint pains. Loose stools with intermittent bleeding goes in favour of ULCERATIVE COLITIS. Management- 1.Soft and bland diet with high protein 2.A PPI 3 .Amino salicylates like mesalamine or sulfasalazine oral and in combination with topical medication by enema or suppository 4 Considering arthropathy steroids to be added Other options are Immunomodulators like azathioprine And Biologics like infliximab

Dx-ulcerative colitis involving rectum sigmoid colon and desending colon.it is inflamatory bowel disease of unknown origin.biopsy of the colonic and rectal mucosa confirm the dx.yet colonoscopic picture with multiple ulcer with inflamed mucosa is suffice to know the diagnosis.Rx-1-Tab sulfasalazine 500mg mg 2×3 8hrly2-Mesacholine suppository 1gm/day 3-Tab b complex 1od till symptoms subsides then mantinance dose of sulfasalazine to prevent complcations.

Dx-colonoscopy suggestive of inflamatorybowel disease ulcerative colitis of unknown cause.it can be confirmed by biopsy of colonic mucosa.it is mild to moderate severity involving rectum and desending colon upto splenic flexure.Rx-1-Tab sulfasalazine 500mg 2×32-Mesalamine suppositeries 1gm/day3-tab b complex 1od till the symptoms subsides then maintance dose foreever.regular follow up of bowel and therpy must be done to exclude coplcations.

Probably ibd uc-moderate proctosigmoiditis Biopsy will confirm Arthritis probably ibd related Stool culture crp Antibiotics Sazo

Most possiblity of Ulcerative Collitis Biopsy for Confirmation.

Multiple ulcerative lesion with joints involvement indicate ulcerative colitis. Symptoms since 3 months weight loss may not be present.
