neonatal phototherapy facts?
day 4 wt 3.7kg serum bilirubin 18.1 tlc 9100 crp negative fever 100.8'F phototheray recommended in fever or how to manage the case?
Fever isnt a contraindication to phototherapy . Admit the child . As crp is negative. As it is summer time . Fever might b due to overbundling of baby . Start double surface phototherapy . Its physiological jaundice . Maintain hydration via iv fluids. Continue breast feeding. Paracetamol for fever .
As on 3rd day if child wt.>3.5 kg,& sr Bilirubin is 18 mg, then double surface Phototherapy is needed, & sr.bilirubin assessment daily should be done. As temp is raised, Admit,& adv.for Sepsis screening and maintain hydration.
This condition seems to be obstructive type due to cholestasis which was developed after the acute viral infection. Treatment will be according to sakhasrita kamala. In my opinion the treatment planning can be done as follows : - Triphala kwatha with Gomutra for nitya virechana. - Arogyavardhini vati - Phalatrikadi kwatha - Bhumyamalaki swarasa
It seems to ne Neo natal jaundice. Sunlight exposure or phototherepy is advised.
See the weight loss if any or signs of dehydration. If crp is negative can repeat it or send PCT. Yes have to give phototherapy, monitor temperature, start antipyretic sos. Blood culture must be sent and take proper history which might give a clue for sepsis.
Might be dehydration fever due to inadequate breastfeeding if sucking & activity good. Look for mother & baby blood group. DSPT & repeat S. Billirubin after 24 hrs
Admit the patient and start double surface phototherapy
