Multiple Mets Supra and infra tentorial regions

New case. 58 yrs, F, CA Lt breast, mastectomy done 2 yrs ago , undergone post op radiotherapy& chemotherapy , on regular follow up with RCC developed progressive unsteadiness 2 weeks, intermittent vertigo wih vomiting 5 days. on exam positive findings include gaze evoked nystagmus with gait ataxia with gr 5 /5power. Opinion of the case.

Thanks Curofy and others who answered the case. T2 / FLAIR iointense lesion both cerebellum and both frontal lobes with intense contrast enhancement with perilesional edema. Ipm : Multiple metastasis. Ref the case to Oncologist further management.

All clinical features suggestive of cerebellum involvement Possibility frontal lesions are silent till now or needs detail evaluation for change in personality , behaviour or cognition

Multiple Cerebral & cerebellar hyper intense SOLs with significant perilesional oedema. Features are consistent with secondaries. Cerebellar CP angle lesions explains vertigo

It's metastasis. Can be considered for radiotherapy/chemotherapy. Prognosis very poor.
