paraphimosis
Newly married young male, presented with paraphimosis, i have tried with puncturing with needle, but still no relief, what could be done please guide

Paraphimosis is a urologic emergency, occurring in uncircumcised males, in which the foreskin becomes trapped behind the corona and forms a tight band of constricting tissue. Often iatrogenically induced, paraphimosis can be prevented by returning the prepuce to cover the glans following penile manipulation. A complete circumcision, or removal of the foreskin, may be necessary in very severe cases of paraphimosis. This will also prevent the condition from happening again. Practicing good hygiene and cleaning the tip of the penis regularly.
It is paraphimosis with long duration with fairly oedematous of prepucial with constriction band which is causing more swelling and this causing difficulty of reduction and if left unattended it may lead to complication blood supply of the glans and prepucial skin. Reduction abd repositioning if the prepucial skin by giving inj hyaluronase locally and by making a few punctures of the oedematous part aseptically and try to squeezing all fluid out then try to reduce the prepucial skin back to normal position. If failed go for dorsal slit and reduce the prepucial skin Later can be planned for circumcision.
? PARAPHIMOSIS.. NEED'S.. TAKE MULTIPLE PRICKS WITH.. STERILE NEEDLE AND SQUEEZE WITH.. GUASE PIECE.. LET THE FLUID COME OUT.. RETRACT THE FORESKIN .. BS..ANTIBIOTICS WITH NSAIDS AS PER REQUIREMENT..

First response is send to Urologist Second response is do under anaesthesia whatever you plan to do. Third Multiple needle punctures with 20 g needle and let the fluid come out. Can aid the process by squeezing on the distal half of penis with circumferential pressure over a pad. Give rhis 5-10 min. minimum. Once the oedema subsides, pull the prepuce back over the glans. Should be successful in more than 90%cases. In the event it is not , proceed for a circumcision
