?Non Healing Ulcer

A 35 year old male is having such lesion shown is pic over his ankle area along with the Arterial Doppler of right lower limb report as attached Investigations Colour Doppler of right lower limb attached Management He is currently on following medicines - 1) aspirin 75 mg 2) Amoxicillin Clavulanic acid 625 mg TDS 3) Pantoprazole DSR OD 30 mins before bfast 4) Tab Cilostazol 50mg twice daily, and 5) Xanthiol Nicotinate 150mg twice daily What would be the best line of management for him next?

Is he a Diabetic or Smoker? Cilostazole before or after food? Thoroughly debride & deslough the wound. Daily dressing with 'WOXheal'. Better results can be obtained if peripheral vascular angioplasty done.

Chronic non healing ulcer foor Arterial color doppler shows thrombus in popliteal artery , compromising arterial flow in popleteal arteries. Suggested Vascular surgeon opinion Endarterectomy followed by NOAC. Statins and Dual anti platelets will be needed for long term follow up.
