Oligomenorrhea since menarche

Chief Complaint A 20 y/o obese female presented with the complaint of oligomenorrhea. She had this complaint since menarche. LMP was 3 months back. History No past medical or surgical history. No relevant family history. Vitals BP: 125/80 mmhg, Pulse: 76 bpm, Breathing: 20 bpm, Temp: 98.1 degree F. Investigations USG shows polycystic ovaries, a normal sized uterus and endometrium thickness: 20 mm. Blood test and hormonal profile normal. Treatment What is your opinion on further investigations and treatment?

Do LH, FSH , Prolactin levels. Now as she is amenorrheic give her withdrawl bleeding with tab primolut N 5 mg bd for 5 days. Advice her on diet and exercise. Advice to avoid spicy, oily junk food,. Drink 3.5 litres water per day. Daily 30 min exercise as per her capacity. She wil require 3 cycles of Krimson 35 od

PCOD. Not mentioned hormonal values. S.Fasting Insulins. S.AMH./S.LH/ S.FSH/ S.Progesterone/ S.Estredol.E2/S.Testestorone. Family history./ Thyroid profile.. If these are all normal than laproscopic4 intervention is required.

This is a case of adolescent pcos. First line of treatment is lifestyle modification with weight loss achievement. First line of pharmacotherapy is combined oral contraceptives with or without Metformin.

Treat as PCOD but must send endometrial biopsy as it's thickened.

In obese patients with PCOS, the best treatment is weight loss. Even a 5 - 10 percent loss in weight, will decrease insulin resistance and normalise the menstruation to an extent. Metformin can also be added.
