Onychomycosis with chronic paronychia Chief Complaints Blackish discoloration of nail with pain and swelling around the nail of forefinger since 6 months History Patient is doctor by profession, repeated use of gloves and repeated hand washings , No history of trauma Vitals Stable Physical Examination Hyperkeratotic hyperpigmented nail with tenderness surrounding the nail of forefinger Investigations KOH Mount from nail scraping Diagnosis Onychomycosis with chronic paronychia Management Cap. Itraconazole 5 mg/kg/day for 1 week in a month for 3 consequetive months Topical antifungal- ciclopirox nail lacquer 8% for 3 months Antiinflammatory tablet for reduction of swelling and pain for 5 days
