Pain at first finger of left hand

A 56-year-old woman was presented with pain at first finger of left hand, swelling and bleeding. Ultrasound and MRI reports attached. Plz, suggest the Dx and Rx for the same.

ONYCHOLYSIS with paronychia. Antihistamine orally. Antibiotics orally. Analgesic ORALLY. Antifungal orally and lulliconazole fucibact oint locally. Dusting of keto power and keep the area clean and covered with sterile gauge.
