Pain in abdomen & fever
45/y/o female with gradually worsening, generalized abdominal pain and tactile fevers for the past 2 days. The pain started in the right lower quadrant but has spread throughout her entire lower abdomen. No h/o vomiting, diarrhea, dysuria, or abnormal vaginal discharge. H/O renal-lithiasis 2 years ago Vital signs are normal. Physical examination reveals bilateral mild lower abdominal tenderness that is slightly more severe on the right. No costovertebral angle tenderness is present. Complete blood cell count reveals a leukocyte count of 19,000/mL. Basic metabolic panel is normal except for a sodium level of 132 mEq/L. Urinalysis reveals a red blood cell count of 25 x 106/µL.
? RENAL PATHOLOGY.. RENAL COLIC.. ? APPENDICITIS.. NEED'S.. USG..ABDOMEN.. SURGEONS OPINION.. MEANWHILE TREAT SYMPTOMATICALLY WITH.. INJECTION.. DYNAPAR..OR.. TRAMADOL..
Dd- rt sided pain indicate that might be Appendicitis, rt ovarian cyst ,rt kidney stone ....bt here symptoms promptly indicate towards the appendicitis....just ask for USG...confirm the dx thn go for further line of treatment...

Pain reduced. Then done usg. Reports r attached. Ovarion cyst n PID. Given doxy 100 bd/continued cataspa ondem n pantoprazol. What antibiotic should be used ofloxacin 400/ ceftriaxone iv/im or n e else.. For the complete relief? Still there is mild pain.plz discus the case...
