Painful lesions over eye -redness since four days and lesion since morning today - no history of travel or steroid use - came for further management ?
Dx :- Herpes Zoster Ophthalmicus . Herpes zoster ophthalmicus occurs when the varicella-zoster virus is reactivated in the ophthalmic division of the trigeminal nerve. It represents up to one fourth of all cases of herpes zoster. Clinical presentations:- This is caused by Human Herpes Virus type 3. Most patients with herpes zoster ophthalmicus present with a periorbital vesicular rash distributed according to the affected dermatome - as in this case . Complications :- A minority of patients may also develop conjunctivitis, keratitis, uveitis, and ocular cranial-nerve palsies. Permanent sequelae of ophthalmic zoster infection may include chronic ocular inflammation, loss of vision, and debilitating pain. Rx :- Antiviral medications such as acyclovir, valacyclovir, and famciclovir remain the mainstay of therapy and are most effective in preventing ocular involvement when begun within 72 hours after the onset of the rash. Timely diagnosis and management of herpes zoster ophthalmicus, and timely referral to an ophthalmologist when ophthalmic involvement is present, are critical in limiting visual morbidity.
Herpes zoster opthalmicus Rx. Valacyclovir 1gm tds for 5 days Cap.pregabalin 75mg with mecobolamine 1500mg od.. Tab.Cefixime 200mg bid NSAIDS antipyretics sos... Look for complications like IRIDOCYCLITIS... CORNEAL INVOLVEMENT... If present... immediate opthalmic opinion needed.. @Dr. Neha Mehrotra mam...need your opinion..
Herpes zoster
Herpes ..... Start acyclovir 400mg qid. Local application of acyclovir 4 to 5 times a day.... Keep watch on cornea. Hutchison's rule.....when tip of nose is involved. ...cornea is almost always involved.

@Dr. Parshuram Agarwal Herpes zoster, be careful take care of eye use herpid eye oint.and use i/v acyclovir immediately.

A male patient of 68 years age, with past medical history of varicella in the childhood, has come to O.P. complaining of photophobia, with lacrimation, severe edema of both eyes, accompanied by clear watery vesicles and scabs in skin of the forehead and nose. The ophthalmic exam showed severe conjunctivitis, keratitis. Hutchinson's sign is positive. Please give the Dx and Rx?

12 yr child developed fever 6 days ago which lasted for 2 days and ranged from 100 to 102 F. On 3 rd day of fever he developed rashes around moth. Now on 6 th day patient is afebrile, with lesions around mouth and congested eyes. Systemic examination is not contributry. Kindly suggest diagnosis and treatment .@
