Painful mass under arm

Chief complaint Painful mass under his right arm. Description A 59 y/o male complaints of painful mass under his right arm No other complaint. Physical examination shows a firm mass of 3.0 cm * 2.0 cm. Past history PMH of HTN & melanoma. Surgical history: He has h/o surgery of melanoma on the right shoulder. CT was negative for metastatic disease at that time & appendectomy also. Vitas BP: 124 / 62 mm Hg. Rest of the vitals are normal. Lab reports CT images attached. Diagnosis Need your opinion on diagnosis.

It is likely Melanoma removed was probably malignant as present lesions seen in axillary area is likely secondary metastasis in lymphnode Liver also shows hypoechoic lesion which need to subject for liver biopsy as it may be mets or primary

Looks like lesions in lt lobe of liver possibly metastatic axillary nodes Pets can ct3 liver core biopsy hp malignancy of liver with metastasis AFP

Afp more then500 is indicative of liver tumour

Suggestive of CARCINOMA NEEDS FURTHER INVESTIGATION AND EVALUATION TO CONCLUDE AND LINE OF TREATMENT.

It is definitely secondary metastasis to axillary lymph node and liver metastasis due to melanoma.
