tonsillitis?

patient is 34yr male,with chief complaint of enlarged tonsil as seen in the image,which ws accidental finding,and reports no symptoms of pain,fever,sore throat or any other difficulty(swallowing and all).. patient is a chain chewer of tobacco for last 6-7 years.. what is the diagnosis? note:one side tonsil is enlarged more than the other one.

Unilateral tonsil enlargement in an adult without any symptoms can be a normal varient or a chronic tonsillitis but more importantly may be a carcinoma of tonsillar fossa. A through examination of neck along with USG needed to check for lymph nodes. If not found can go for a deep FNAC from tonsil is needed. If still negative follow up patient up to 6 months .......... If any signs are positive Radical tonsillectomy with or without neck dissection with radiotherapy after biopsy.

If it is recent onset and/or progressive in size, then the following d/d must be ruled out: 1. Parapharyngeal tumour 2. Tumour of deep lobe of parotid gland 3. Lymphoma (NHL) Though the appearance of tonsil does not indicate Ca Tonsil, nevertheless a biopsy can be considered, especially if there is coexisting positive cervical lymphadenopathy.

it's due to tobacco use over long time. nicotine and other chemicals affects the immune response of mucosa and also change the micro flora of the oral cavity. means harmful microorganisms gets the chance to flourish. tobacco causes decreased immune response and inflammation side by side . there are fair chances of tonsilolith occurrence. Rx: cessation of tobacco and mouthwash
