Dear dr Kamini, U r in dilemma mainly bez she is unmarried Wt loss can be bz of decreased appetite due to pain &anxiety. Get her routine haemogram done urine routine, & mainly Ca 125 Then if required u can go for USG guided fnac from the ovarian cyst. As a last u have to go for diagnostic & operative laparoscopy.

R/O for Pid in this case.Take Tb gold value, Mx. test always shows negative response on genital Tb, simple ovarian cyst might b functional, take post menstrual USG findings. Ca 125 is also advisable

since h/o weight loss is present, it could be tubercular. Investigate for pelvic tuberculosis

when was the LMP and what are the CA-125 levels

the ovarian cyst is small to be .ca...So it's btr to be on the safer side n do the tumour markers..she is only 21yrs so only ca125 wouldn't be enough so she needs hcg,ldh,Alfa feto proteins..N this could most probably be dermoid or a endometriotic cyst ..N she needs a btr scan

what was the LMP?? start crimson 35 1OD for 21 days from 5th day of menses. for 6 month tab ovaries SR 1OD for 6mnth.. for heathy ovaries n syp evecare/ menohelp 2tsf OD.. (utrine tonic) tab meftal spas 1BD for 2 days..

PLEASE MENTION LMP. Adv. CA 125.if it's normal go for ocps for 3months.if still not subsided, we can go for laptops scopy.endo me trial tissue for TB PCR. And adv viral screening.
