Treatment of hepatitis B positive patients depend on activity of the hepatitis B virus Hence any thing that shows damage to hepatocytes would warrant treatment for hepatitis B Activity is measured by 1) Hepatitis B viral load 2) Liver enzymes - persistently raised liver enzymes would indicate continuous damage 3) Fibroscan or ARFI scan to look for fibrosis in liver - any degree of fibrosis would need treatment Treatment is by Antiviral medication Such as Tenofovir alefenamide Entecavir Caution Before starting any treatment rule out HIV 1 AND 2

Give inj hepavx twice weekly for 6 months Immunoglobulins SC or I m

Usually there is a natural clearing of HBsAg from blood ,in around 6 months . Better assessment of disease activity can be done by checking Viral load ( DNA copies) , and elevation of Alkaline phosphatase in blood, and whether there is evidence of Cirrhosis . Hepatologist is to be consulted . Antivirals like Tenofovir, and Lamivudine need to be used if HBsAg is persistent or patient is symptomatic. HBsAg negative contacts are to be immunized with Hepatis B vaccine .
