First of all i congratulate sir, for work u r doing in ur field. u r fullfilling the dreams of such ppl who has deformities.. im truly humbled by ur work love ,determination,affection towards it. great salute sir!!!!! lets come to case: -Report: -loss of lumbar curvature -osteophytes-dessication disc -bulging and herniation disc report suggest: -ptn in severe pain which has caused spasms in paraspinal muscles -neurological deficit is present with sensory and motor functioning. ankle joint reflex -Disc is in extrusion or in protrusion phase.[L4-L5-S1] impinging on nerves exit.Tear of annulus fibres. -this above abnormal posture is due to neuroforaminal stenosis. Advise: -My personal assessment from reading it looks further ptn may require surgery-Dissectomy. -conservative management with physiotherapy can be planned. like NSAID with muscle relaxant with corticosteroids. in physiotherapy: TENS-SWD-TRACTION

Central discs herniations.. These provoke low back pain and saddle pain in the S1 and S2 distributions. A central herniated disc may also compress nerve roots of the cauda equina, resulting in difficult urination, incontinence or impotence.. Coz w.hen the disc extrudes into in the spinal cord. Subarticular (Lateral Recess or Paracentral): When the disc extrudes between the spinal cord and the foramen (the space through which the nerves exit the spinal canal). Foraminal (Lateral): Disc extrusion into the foramen.. Spinal surgeions have latest theories of mansgements. Pl.consult him.
