a 28yrs female complains of itching under such lesions in various region on body since 10yrs and she has constipation also. h/o amnorrohea 2 mnths. plz dx n t/t

this type of since 7 years itching+++ pt taken lot of antifungal and applied lot of anti fungals ointments but no relief

A patient aged 44 yrs came with skin erruption as shown in picture with much itching, feels good to scratch followed by blackish disquamation started first on legs now spread to whole body, but now disappeared from legs < summer, rainy, sweating, contact of water> winter. (h/o present company given in past history) His 2nd compliants- Feeling of occasional sudden great weakness, oppressed feeling as if air is insufficient to meet his inspiration< while talking, associate with dryness feeling in throat. 3rd compl- tingling numbness left fingers for 4-5 months 4th complaint - strain at urine, few drops on clothes after passages. 5th - stoppages feeling noses. o/e DNS with swelled turbinates 6th - indigestion, flatulency 7th - urging stool early morning with mucoid stool Family history- father - h/o leprosy with residual symptoms, heart block operated, GB stone, gastritis, Very much sensitive to open air must cover head and face to prevent headache and rhinitis, profuse sweat when just approaches and during taking food mother-in-law hypertension, maternal uncle - asthma, diabetic, hypertension Past history- ricket early childhood. At the age of 18, in 1991 asthmatic trouble, took Sulphur 0/1 to 0/10 in 4 months from some homoeopath with relief of asthmatic problem but appearance of this skin erruption on legs. To get relief of skin trouble after a year of suffering went to dermatologist, again asthmatic problem started when external ointment applied. he stopped allopathic treatment and went to homoeopath with relief of asthma but skin problem spread whole body where he got NAT MUR, PULS, MERC SOL, PSORIN. With psorinum erruption on legs disappeared. Generalities- desire - meat, no desire to salt av- bitter thirst - moderate tongue - flabby, dry shallow crack in the middle Perspiration- profuse whole body sleep- deprived thermal r- sensitive to both extreme mind - extremely nervous, anxious, fear of ghost, quarrel, news of fighting somewhere, Lab test - USG - fatty liver, mild prostatomegaly, renal lipodystrophy?/angioma? Hba1c - 9, TSH - 6.9, Taking allopathic anti diabetic medicines Depending on above I gave- 05/05/2016 - Bacillinum 200/ 4 doses, weekly once, Kali bichrome 30/4 doses OD 08/06/16- Bacillinum 200/ 8 GLOBULES in liquid to take OD, and Kali bi 200 / liquid dose BD as mild > but again < 22/06/16 - no ameliorate Started MERC SOL LM pot UpTo 0/10 on 19/01/17 in between I gave Bell for common cold, Stann met30/ 1 dose for acute attack of chest discomfort his 2nd compliants- which relieved him promptly, Mezereum as veg analog to MERC SOL, and Bacillinum 200 intercurrently 06/02/17- I change the medicine as patient reported this winter his erruption didnot disappeared as before and he is anxious of comming summer. Ars Alb 200/ 6 drops OD continue, Arnica months 30/ 6 drops BD continue both prepared in 15 ML dist water one drop of medicine. the out come after giving medicine regarding skin problem shown in photos given date wise What should be my approach to this case now? management, Rx, dx

35 years Mel patient, Sevear Itching,than burningness,duration 2 years, no others symptoms and history. please doctor treatment and management.
